[fusion-dev] fusion 1.0.4 release

This is a quick release to consolidate the SVN screw up stuff into a  
stable release that we can use to move development forward.  Nothing  
different from 1.0.3 except for perhaps a few miscellaneous things  
that came together through the merge.
