Comment:

 I suspect that the very small scale is the issue here (extent interpreted
 as meters gives a total extent of about 5m x 5m).  Also, the large values
 for the OverviewMap min and max ratio parameters mean that the overview
 map is zoomed out much too far so rounding errors will throw off the
 rectangle.
 Closing this for now.  If the problem persists with correct units for the
 map definition and better min/max ration parameters please re-open the
 issue.
