Folks,

For reference I have incorporated Pirmin's patch to support
reading XYM shapefiles as XYZ.  Comprehensive XYZM
geometry support would be an interesting objective for GDAL/OGR
2.0.

I also fixed the JP2KAK overview bug mentioned in this thread.
