[gdal-dev] Generating 200x200 tiles

Hi Alistair,

You could try ctb-tile 
(https://github.com/geo-data/cesium-terrain-builder#ctb-tile) with 
the`--tile-size 200` option.  This uses GDAL under the hood and works on 
GDAL datasets, so pretty similar to gdal2tiles.
