Jay,

The printpdf2 (reporting style) only works in landscape mode.  You can edit the service definition in the mapbook to make the "options" go away.

One last mention on the printing….
 
I got it working although when I
choose Landscape, No north arrow and no Scale bar they still show up on the
map.
 
Is there something else that I
am doing wrong….
