I unzipped the demo app and it works normally. Then I made a copy of it under a new name. When I open this application, all folders are expanded. They are both residing in the same ms4w/apps directory, the only difference is the application folder name. I also checked and your getmapbook.php is returning the mapbook you sent in the email.
