This looks like this bug:

It is fixed in GRASS >= 6.1.

GRASS 6.02 is very old. Please use the latest official stable 6.2.1 or
the 6.2.2 snapshot, preferably.

Also, for convenient digitizing, I recommend the GRASS Edit tool in
QGIS 0.8, over the v.digit.
