  Remove old style CGI runtime substitutions

This implements mapserver/mapserver/#4604.  All runtime substitution
configuration is now performed using `VALIDATION` blocks in `CLASS`,
`LAYER` or `WEB`: `METADATA` blocks are no longer involved.
