Not to jump in here, but if this is bug 1322, it does need a caching 
system to solve it.  It simply needs a simpler, more secure method of 
naming temp files (mkstemp on Unix and GetTempFileName on Windows are most 
likely).  Daniel and I had discussed killing this one right after 4.6.

Not that caching images in Mapserver is a bad idea (caching individual 
layers could be a huge improvement that can't be done in squid).
