Just a hunch, but try adding 

  UNITS dd

after your imagecolor line...

I have gotten the demo to work pretty well, and I am now attempting to
develop my own images, but am running into an issue at this point.  The
initial image that it creates is not even close to the actual extents,
and is not able to load into the applet window.  The code initial lines
in the map file are:

NAME FLORIDA
STATUS ON
SIZE 600 600
EXTENT -78 24.2 -88 31.2
SHAPEPATH "data
IMAGECOLOR 255 255 255

IMAGETYPE PNG

PROJECTION
  "proj=latlong"
END

I have attached the file to give a better understanding.  I am guessing
it is something simple, but it has dumbfounded me for most of the
morning...
