laodForm() error

You should have exactly 4 values for any extent value. This error means you
had more or less. Typically you see it if:

  - the extent is provided using a delimeter other than a space
  - the extent is not properly escaped in the URL (spaces must be turned into +'s or %20's)

Check those first.
