Blank Template

My ka-map is working fine,
  i am running query thru mapserver CGI file

  Every thing is working fine but the query displays the template file with no result it just print the file.here is my query output. why it is not printing the values from dbf file.

   code no   
   name

  here is my template file.

  <body>
  <li>code no [codeno]
  <li>name [name]
  </body>
  </html>
