Hi, 

All I can say is that I am running MS4W 3.0.2 updated to use Mapserver
6.0 and I have these two lines in the LAYER-METADATA for setting the
proxy and it works for me.

"wms_proxy_host" "xxx.yyy.zzz"
"wms_proxy_port" "8888"
