I'm not familiar with mapserver, but I would highly recommend you get 
used to the using either firebug or chrome developer tools to help get 
yourself on the way. In this case, you want to look at the NET tab in 
either and look at the URLs (and the response) that the code is 
generating. You can cut/paste (or just open) one the URLs directly in 
the browser. Working correctly, it would give you an image but I am 
guessing it is generating malformed URLs which should give you huge hint 
about the problem.
