[pgpointcloud] PostGIS extract for pointcloud

https://github.com/pramsey/pointcloud/ has a basic example for fetching data. Search the page for "drivers.las.writer". There isn't too much else for examples, but https://github.com/PDAL/PDAL/tree/master/test/data/pipeline is a trove of other operations that you might be able to dig through to figure out how to snap stuff together.
