[postgis-devel] Making a table in appendix doc for upgrade version

A table like that would start very large and keep growing...
The Release Notes appendix contains the informations already
just read the section of the release you're going to install.

We should probably refer to the manual from the README.postgis
file rather than have duplicated info spread around...
