 The vrt that I attached is one generated by raster2pgsql when it is
 processing the source raster.  Normally it is only in memory (not a file)
 but I tweaked the raster2pgsql loader code to spit out the VRT as a file
 so that I could inspect it.

 After doing a debug build of PostGIS and testing the loader, I can't find
 any problems with the entire loading process.  Testing other tiles...
