If you installed PostGIS via stack builder, the easiest way to create a
PostGIS database is to 
 
create a database and use template_postgis as the template.

If you are trying to spatially enable an already existing database, then
make sure you have plpgsql language enabled in the database before you
install the postgis scripts.  You can do this with a 
 
CREATE LANGUAGE plpgsql;
 
I think PostgreSQL 8.4+ has it installed by default at least for windows,
but 8.3 may not.
 
We take it from your below that you are trying to install PostGIS 1.3
instead of the newer PostGIS 1.5. 
For PostGIS 1.5 and 1.4, the file you need to install is postgis.sql (prior
versions was lwpostgis.sql).
 
If there isn't any particular reason why you are using an older version, I
would suggest to use PostGIS 1.5.  It has a lot more functionality than 1.3.

i am a new developer who use a mapnik to build my own map. I use Window
instead of ubuntu.I want to know how to install postgis on windows?
