Using I use MapServer to generate either a png or a jpg at a resolution
of 150 or 300dpi and then embed that into the pdf using FPDF, I do not
bother with the pdf output format with MapServer.<br>

As a side note, you can use pdftk (open source) to overlay, merge, and
fill PDF forms/documents. If you wanted to use MapServer PDF's to
merge into reports this would probably be the way to go.<br>
