Check if you have QtSql4.dll or any other Qt*.dll in C:\Windows\System32 or
another folder that is in your PATH.

In Portugal, there is a Government application that supports legal digital
signatures that places Qt4 dlls in the System32 folder. This causes problems
with other applications that use different versions of Qt4.
