My suggestion isn't an open source one (but built with OS
Components) - take a look at Fulcrum. Offline data collection and it
will do about anything. I use it in conjunction with QGIS and
PostGIS for data collection. While not free it does make life a tad
bit easier in the data collection realm...and super friendly to qgis

It's not QGIS but Geopaparazzi is a good tool to collect data in
the field with a tablet. You can test it from Android Playstore
easily.<br>
You can load maps in mbtiles format (without internet connexion)
or "in the fly" if you can connect to a WMS server, and you can
easily define your own form to collect data (with data validation,
photos, etc.)<br>
