It's not QGIS but Geopaparazzi is a good tool to collect data in the
field with a tablet. You can test it from Android Playstore easily.<br>
You can load maps in mbtiles format (without internet connexion) or
"in the fly" if you can connect to a WMS server, and you can easily
define your own form to collect data (with data validation, photos,
etc.)<br>
