[fdo-users] Write FDO Provider

Your entry point would be something like:

extern "C" MY_API FdoIConnection* CreateConnection()
{
     return new MyFdoConnection();
}

This is the function signature that FDO is looking for. MY_API would be a
#defined symbol like:

#ifdef _WIN32
#define MY_API __declspec(dllexport)
#else
#define MY_API
#endif
