Dan,
The behavior you are seeing is surprising to me. I'd like to suggest you
try reproducing the problem with a small input file using FWTools 1.3.1.
If you can reproduce it there, then file an appropriate bug report against
GDAL, otherwise we can try to isolate what the differences are.
