I think this is a terrific start!
 
To bring this up to the capabilities that I already use from FWTools, how would an end user like myself add in support for additional formats (e.g. JPEG2000, MrSID) and .NET bindings?

That's pretty close I think.

The nsi could need some finishing touches (version handling on upgrades and
some artwork).  But apart from that it seems to work.  You get a desktop link
and a start menu entry that both opens a command line window from where you can
use GDAL and start python with gdal available.

But that's as far as it gets with repacking OSGeo4W.  With that we don't
support different/other pythons, perl, compilers...
