For MS4W (http://ms4w.com) we build GDAL 2.0.2 against 
libspatialite-4.3.0 and sqlite-3.8.10.2

I haven't heard anyone from the MS4W community report of the errors you 
mentioned.  I have myself used ogr2ogr to create a valid GPKG format though.
