[Geo4All] website feedback survey

We should be sure to include this feedback in the feedback survey form, I
imagine we will use the resulting report to set development priorities for
the coming year.

We have a had a couple of great ideas on the GeoForAll list that we did not
end up getting into the issue tracker for discussion (or arrived too late
to be implemented.) This feedback form is a good opportunity to take part
in the planning for subsequent improvements.

I note that budget requests have gone out for 2018, given the large number
of ideas generated by this list the GeoForAll committee may wish to request
a budget (to set up a small contract) to explore and implement some of the
ideas and enhancements that missed out on this years website reboot.
