This one's a quicky I can address (very busy today):

In the mapbook:
<group title="My Group">
    <!-- Put the MAP Entries here -->
</group>

Optionally you can have an attribute "expand" that when set to "true" will have the group open "expanded" already.

<group title="My Expanded Group" expand="true">
</group>

You can also next groups:
<group title="Outside">
    <group title="Inside">
        <group title="Inside Inside">
    </group>
</group>
</group>
