Try checking the mapfile_root parameter in the mapbook and root in settings.ini.  I believe both should have trailing slashes.

I also got only blank pages when printing (GM 2.0.1, no matter what 
quality). The resulting files are of 0kb file size.

I put "MAXSIZE 10000" in my globals.map as suggested earlier by Len.

Where (which file) and how can I use "printf" statements?
