A green banner and no catalog is a classic mapbook problem. Either the mapbook isn't getting sent to the browser via the PHP script or it is invalid. Try looking at the getmapbook.php request in a web browser and see if it looks ok.
Also, I'm not sure what the status is with the PHP that ships with GeoMoose 2.4 with regards to MapServer 6. I know there were some changes that were needed to make it all work together on Linux. (GM 2.4 predates MS 6 and there are some major changes/improvements in that version that cause some incompatibilities.) GM 2.6 will expect MapServer 6.
