A few thoughts: At least the demo works and my installation works, so it must be something you did that is different. It could still very well be a bug, but unless we can figure out what is different, we have little hope in fixing it.
Is it a nested group? I haven't personally tested that.
Are you sure you are running the code/mapbook you think you are running? (I know it's silly, but I've been bitten by this more than once.)
Can you send us a link to the app and/or mapbook?
