Are you getting anything at all for that layer? For instance, are the
results showing the title of the layer? Is the layer on, visible, and
within scale range when you try to identify it?

Thanks for the suggestion. This is what I have in the mapfile metadata
section. I am not sure if I used it right or not. It still does not
work
after I added fields to my mapfile metadata.

You don't have the "fields" item in your metadata section. Example
(belongs
in Metadata section of map file):
"fields"
"FID:FID,RDNAME:RDNAME,L_ADD_FROM:L_ADD_FROM,R_ADD_FROM:R_ADD_FROM"

I have the same issue before, first thing i would asked to you is :
(this is my stupid mistake) are your identify template file
"identify_placenames.html" in the same folder with your mapfile?
Then, make sure there is no missed typed names, attribute column, etc.
