Bob, I have been using ArcMap to create that template PDF and it seems to work fine. Maybe it's something with fpdf and Word.
I do agree with Mark that fpdf might be a bit dated and maybe there are better options to consider in the future. Another option would be to get rid of the PDF creation and just use a html page that is formatted to be printable. Most people have options these days to print to pdf from their browser.

Hello,
In the past I think I had success using primo pdf to create the template. However that was a while ago so things could have changed...

I'm trying to modify the default PDF printing templates for GM 2.6.1, adding my own agency logo. I do the process by making a copy of the template into Word, make the modifications, them use Word's Save As Adobe PDF function to save it back as a PDF file.
When I attempt to use it, I find an error in Firebug:
<b>FPDF error:</b> Unable to find xref table.
This sounds like an error from the underlying pdf library. It seems that your pdf has some feature that is not supported by fpdf. Try taking the pdf you made and running it through a pdf printer or otherwise converting it to pdf with a different method.

I've followed the GM documentation and think I was doing it correctly. Suggestions?
