Jeff,

Thank you for offering to take a look.  I think there may indeed be an issue with MS4W not being able to read DB files.  I put together a test file located at http://geomoose.lyonco.org/DBTest/DBConnectionTest.zip
 
*Unzip the file to /ms4w/tmp/ms_tmp
*There is a test url bookmark that will open up http://localhost/cgi-bin/mapserv.exe?service=WMS&version=1.1.1&request=GetMap&styles=&format=image%2Fjpeg&MAP=%2Fms4w%2Ftmp%2Fms_tmp%2F%2Fselect_132761447356573.map&FORMAT=image%2Fpng&TRANSPARENT=true&LAYERS=highlight&SRS=EPSG%3A3857&_OLSALT=0.5711829908262914&BBOX=-10375479.594299,5556082.9582284,-10370821.712889,5558810.804675&WIDTH=975&HEIGHT=571

*I included a shapefile variation of the mapfile to demonstrate that shapefiles work.
