Solved this, thanks.  For the benefit of anyone reading the list 
archives, from the command prompt I had to run

autogen.bat
"C:\Program Files (x86)\Microsoft Visual Studio 9.0\VC\Vcvarsall.bat" amd64

And then the usual build instruction worked fine

nmake /f makefile.vc MSVC_VER=1500

A few warnings existed about size_t being condensed to ulong.  Not a 
problem for my purposes.
