  docs: Use GRASS, tools, and no generic headers (#5471)

This replaces use of GRASS GIS by GRASS in generated Markdown, MkDocs, and Sphinx (it leaves custom HTML doc mostly untouched). It also replaces couple remaining words modules by tools.

This removes and simplifies the original overly generic headers - level 1 headings and text about GRASS - by just a specialized heading for the tool category (class, family). It also removes link to index of topics.

Finally, this cleans up some of the string template code for Markdown and replaces some addons-related HTML by Markdown.
