  seo: meta tags and schema fixes

Per-page metadata + JSON-LD (#513, #525)

themes/grass/layouts/partials/head.html (modified): title, description,
and all og:/twitter: tags are now per-page (were site-wide on every page);
fixed the invalid twitter:card value (was the site name) to
summary_large_image; added og:image:alt, twitter:image(:alt),
og:locale, and article:published_time/modified_time on content pages.
themes/grass/layouts/partials/schema.html (new): Organization, WebSite,
SoftwareApplication JSON-LD with the real social links.
