While researching confusion matrices, I came across an excellent online resource
 http://everest.geo.hunter.cuny.edu/~chuck/dissertation/fullDissertation.html  by
Chuck Ehlschlaeger on "Tools for representing spatial application uncertainty".

Very good literature review, excellent command of the topic, and there is a 
link to some of his Grass code and scripts that he used in the 1998 dissertation.
