FYI
Month ago I have submitted a "krovak" update to PROJ4, but it
was not accepted (yet?):

The best would be to raise that issue on the PROJ4 mailing list
http://www.remotesensing.org/proj/

Interesting to know is that the Evenden version of PROJ4 (but without
datum transformation) now contains Krovak support:
