For what it's worth:
If you can't use SG3d instead of d.3d, you're in for a bit of trouble. The two problems you mentioned can be solved by a) setting the center of view and b) setting the point of view. Both point
have to be entered as an x, y, z absolute position. If your map is off the screen apparently 
you did not set the center of view correctly; if it's too far away you should reduce the distance
between the center of view and the poit of view.

In case you intend to create a 3d view "from the surface itself", you will run into some more
trouble. Hope this next description of mine is of some help:
	1) Don't put your point of view too near the surface. This will probably result
in a "view from below" which you won't want anyone else to see;
	2) If you're looking in a particular direction, say due south, use the region and/or
mask to lop off any surface behind you. If you don't, what's behind you may turn up inverted 
and in front of you. Although this results in some nice Space Age pictures it's no use either;
	3) Get SG3d right away.
