Some fixes have been applied recently
http://trac.osgeo.org/grass/log/grass/branches/releasebranch_6_4/scripts/v.db.addcol
which are available now in GRASS 6.4.1.

If you can, please update to this bugfix release. But maybe others
have an idea, too.
