without really looking at it, I would check that the bytes-per-
cell is consistent in both the output and the input.

if r.out.bin is exporting at 2 bytes per cell and xyz2grd is
expecting 4 bytes per cell.. 


also, have a look at r.out.gmt and similar modules from the
GRASS addons repository. It takes care of a lot of this stuff
for you.
