[Lexicon] Glossary maturity levels

Hi Rob (and other Glossarians),

Based on prior conversations, I've sketched out a set of glossary maturity
levels

and proposed fields for each level.

I'm mindful that I haven't linked most of these fields with existing
metadata profiles, and would love some feedback from those in the know.

I've also added a section on "Implementation challenges" to discuss with
Dan when we connect with him.
