[mapguide-internals] Updated: MapGuide RFC 16 - Elevation and Extrusion Support for KML

Following feedback on this RFC, I have updated it with two minor
modifications:

1) I have added the ability to specify the units to use for the
offset/extrusion settings (assuming same units for both).
2) I have moved the ElevationSettings out of the ExtendedData1 element
and into the main schema, since the schema version will be incremented
for the cartographic stylization RFC anyway.

Please let me know if you have any further comments.
