Old description:

 Currently, if a provider is marked as !PerCommandThreaded/MultiThreaded
 then it is treated the same as if it was !PerConnectionThreaded. This is
 inefficent and doesn't take advantage of the extra capabilites of the FDO
 provider if it supports !PerCommandThreaded/MultiThreaded.
