[mapguide-users] Add a new icon on toolbar mapguide

for the custom commands in MGE 2009, this is how it works
under 'url to invoke' it only needs the end bit of the url from
/mapguide2009 (www folder). The backslashes are changed to forward slashes
for some reason.

example:
/mapguide2009/mapviewerphp/feedbackform.php


I'm not entirely sure if this is the same as MGOS but in 2009 it works like
this: to add an icon it's best to save it as a .gif or .png and save it in
the www/stdicons folder then follow the structure of the built-in commands
which will generally have the path to the image location as
"../stdicons/icon_print.gif"
