That's possible.
Do a change and modify the install. Add the .NET stuff.
Configure the mapviewerajax as an application that points to the mapviewernet folder. Make the default doc the ajaxviewer.aspx file. Make the mapviewernet folder an application as well. The install should create a bin folder with the required MG dlls.
I think that's mostly it.

I'm guessing that as long as Apache isn't running on port 80, you could
also perform a repair install on the web extensions to add the .Net
dlls, and then configure the mapguide virtual directories manually in
IIS. There is info floating around there somewhere on how to do this.

Yes. Just uninstall the web tier and reinstall it with .NET. Don't touch
the mgserver install.
