You have not told us what the coordinate system of your layer feature source
is and what the coordinate system of your Map Definition is. 

If they are different, you should be inserting markers using the coordinates
based on the coordinate system of the *layer feature source* and not the Map
Definition, as non-matching layers are transformed on-the-fly to match the
coordinate system of your Map Definition.
