[mapguide-users] FDO Provider SQL Server Spatial and Table-Valued Functions

I've added a sql script to the FDO ticket. Just a table with a geometry
column and a table-valued function to access this table.

After creating the db structure just create a FeatureSource in Maestro and
either push button 'LoadDefaults' under 'Coordinate System Overrides' or
push button 'Spatial Contexts'.
