The problem was due to a bug in Firefox. When the HTML DOM of the page is modified within the onload handler of a page, this page won't correctly notify its parent that it finished loading so the parent page's onload event handler is never fired.

This submission delays the creation of the contextmenu and toolbar menu(s) to after the onload event. The onload event is now correctly propagated.
